The best ergonomic desk adjustments are rarely about buying an expensive new set-up. They are about making a few sensible changes that reduce unnecessary mechanical tension and give your body more opportunities to move.
For desk-bound professionals, neck stiffness, aching shoulders, tension headaches and lower-back discomfort are common reasons to seek osteopathic support. Yet sitting is not inherently harmful, and there is no single perfect posture to hold all day. Our bodies are adaptable. The aim is to make your workstation fit you, then build regular movement into your working day so that no one position becomes too demanding.
Start with the chair, not the screen
Your chair provides the foundation for the rest of your desk set-up. Sit back so your pelvis is supported by the backrest, rather than perching on the edge of the seat. Your feet should rest comfortably on the floor, with your knees roughly level with, or just below, your hips. If your feet dangle, use a stable footrest or a firm stack of books.
Set the chair height first. When your feet are supported, your forearms should be able to rest near desk height without your shoulders creeping upwards. If the desk is too high, lowering the chair may leave your feet unsupported, which is where a footrest becomes useful. If the desk is too low, repeatedly rounding towards the keyboard can place more demand on your neck, upper back and wrists.
Lumbar support can feel helpful, but it does not need to force your lower back into an exaggerated arch. A small cushion or rolled towel may provide gentle support if the chair is flat or worn. Comfort is the useful guide here: you should feel supported, not fixed in place.
The best ergonomic desk adjustments for your screen
A screen that is too low encourages your head to drift forwards and down. Over time, this can contribute to postural tension through the neck and upper back, particularly when work is stressful and breaks disappear from the diary.
Place your main screen directly in front of you, rather than off to one side. The top third of the display should usually sit around eye level, allowing you to look slightly down at the centre of the screen without bending your neck. The exact height depends on your eyesight, bifocals and personal comfort. If you wear varifocals, a slightly lower monitor may reduce the need to tip your head backwards.
Keep the screen approximately an arm’s length away, then adjust for text size and visual comfort. Squinting, leaning forwards and frequent eye strain are signs that the issue may be screen distance, font size, glare or an out-of-date prescription, rather than posture alone.
For laptop work, the most effective change is often simple: raise the laptop on a stand or solid books, then use a separate keyboard and mouse. This prevents the usual compromise where the screen is too low if the keyboard is at a usable height, or the keyboard is too high if the screen is raised.
Use two screens with intention
Dual monitors can be efficient, but they can also create a repetitive neck rotation if one is used far more than the other. If you use one screen for most tasks, place that screen directly in front of you and position the second beside it. If both are used equally, place them close together in a shallow V shape, with the join in front of you.
Avoid placing a frequently used monitor at the far edge of the desk. Small rotations are normal. Repeatedly working with your head turned is less forgiving, especially when combined with a long day, poor sleep or a previous neck problem.
Bring your keyboard and mouse closer
Your keyboard should sit close enough that your elbows remain near your sides. Aim for relaxed shoulders and bent elbows, rather than reaching forwards with straight arms. Your wrists should remain broadly neutral, without being held sharply up, down or sideways.
The mouse matters just as much. Keep it beside the keyboard, not stretched out across the desk, and move it from the shoulder and elbow rather than only the wrist. A mouse that feels too small, too large or difficult to grip can aggravate hand, forearm or shoulder symptoms. There is no universally superior mouse shape, but a comfortable device that suits the size of your hand is worth considering.
Try not to rest your wrists on a hard desk edge while typing. A soft edge support can improve comfort for some people, although it should support the heel of the hand during pauses rather than encourage pressure directly through the wrist while typing.
Make room for the work you actually do
Ergonomics is not just about monitor height. Consider the objects you reach for most often. Your keyboard, mouse, mobile phone, notebook and water should sit within easy reach. If you take regular calls, use a headset rather than trapping a mobile phone between your ear and shoulder.
Paper-based work creates another common problem. Looking down at notes for hours can be more demanding on the neck than screen work. A document holder placed beside or between your screens can reduce repeated head movements. For shorter tasks, simply raising paperwork on a folder can make a meaningful difference.
If you work from home, do not underestimate the value of creating a consistent work zone, even in a small space. A dedicated chair and a clear desk can help your body associate that area with supported working, while making it easier to step away properly at the end of the day.
Change position before discomfort builds
The most supportive chair cannot make eight uninterrupted hours of sitting ideal. Muscles, joints and the nervous system generally respond well to variety. Rather than chasing a rigid, textbook posture, give yourself permission to shift, stand and move regularly.
A useful starting point is to stand or walk briefly every 30 to 60 minutes. This might mean refilling your water, taking a call standing up, walking upstairs between meetings or doing a few gentle shoulder rolls while a document loads. Short, frequent movement breaks are often more realistic than waiting for one long exercise session after work.
Sit-stand desks can be helpful, but they are not essential and are not a cure for pain. Standing still for long periods may irritate feet, legs or lower backs just as prolonged sitting can. If you have a sit-stand desk, alternate positions gradually. Begin with 15 to 20 minutes standing, then return to sitting before fatigue sets in.
Let symptoms guide the adjustment
An ergonomic change should make work feel easier within days or weeks, not create new discomfort. If a raised screen causes you to extend your neck, lower it slightly. If a lumbar cushion feels intrusive, reduce its size or remove it. The right set-up is responsive to your body, your tasks and any existing injury.
It also helps to notice patterns. Do symptoms worsen after video calls, intense deadlines, long drives or poor sleep? Stress can increase muscle guarding and amplify pain sensitivity, while low activity can make stiffness feel more persistent. This does not mean symptoms are imagined. It means recovery often benefits from addressing the wider picture alongside desk mechanics.
